Request a Grant Writer

Grants to Florida, Kentucky, Louisiana, Tennessee, and Texas nonprofit organizations, public agencies, and IHEs in eligible communities to address overall health and wellbeing. The deadline to submit a request for application is April 27. The Foundation invites proposals from organizations addressing the barriers keeping people from overcoming social determinants of health.

For the 2018 annual grant cycle, nonprofit organizations implementing programs in one or more of the following communities are being considered:
-Baton Rouge
-Broward County in South Florida
-Jacksonville
-Knoxville
-Louisville
-New Orleans
-San Antonio
-Tampa Bay

Social determinants of health are the conditions under which people are born, grow, live, work and age that impact overall health and well-being.

Humana’s focus areas for investments in the Southern U.S. are:
-Social connection
-Food security

Humana’s current focus areas for Louisville investments are:
-Post-secondary success and sustaining employment
-Asset security

A strong application should be data-driven, evidenced-based, sustainable, scalable, and financially stable.

All grant recipients must use the Centers for Disease Control and Prevention’s Healthy Days Survey to measure impact along with one or more of the following validated survey tools:
-Duke Social Innovation Index
-USDA Food Insecurity Survey Module

Additional Eligibility Criteria:

Humana Foundation funding opportunities are available to higher education institutions and nonprofit organizations or public entities that are 501(c)(3) tax exempt.

The Humana Foundation will not provide funding for the following instances:
-Sponsorships or fundraising events
-Capital campaigns
-Grants for individuals or private foundations
-Lobbying efforts, political action committees, or politically sensitive causes
-Endowments or reserve funds
-Seed money, or as the only funding source of an organization
-Indirect expenses unrelated to the project being funded
-Religious organizations for general operating support, sanctuary construction, renovation, or mission-focused work.

The Humana Foundation will not knowingly provide support to organizations that discriminate against a person or group on the basis of age, political affiliation, race, national origin, ethnicity, gender, gender identity, disability, sexual orientation, or religious belief.

Pre-Application Information:

If you are a national nonprofit that operates in several of the communities the Foundation serves, you only need to submit one Request for Application. Be sure to list all the communities where you plan to implement your project.

Investment Timeline:

Spring 2018:

-Requests for Application (RFA) will be accepted between April 1–27, 2018 from organizations implementing programs in eligible communities. Please visit the Humana Foundation website on April 1, 2018 to access the link to complete an online RFA.

-RFAs will be reviewed by the Humana Foundation and all applicants will be notified of the status of their RFA by May 11, 2018.

-Selected organizations will be invited to submit online applications, between May 14 – June 1, 2018.

Summer 2018:

-Grant investments will be awarded in the summer of 2018. All applicants will be notified of the status of their request and selected.
